Summary:
54 YOF had been diagnosed with primary hyperparathyroidism following ???routine???
serum calcium determination, in August 2000. Serum calcium was 11.1 mg/dL, urinary
calcium 476 mg/24h, and serum PTH was high (180 pg/ml). Ultrasound of the neck
and CT of the chest did not localize a parathyroid adenoma.
Clinical Information and Procedure:
Planar Tc99m Sestamibi dual-phase scan identified focal uptake in the upper
mediastinum. Tc99 SPECT with Hawkeye performed.
Findings:
Tomographic imaging performed after the early acquisition included CT anatomic
localization via ???HAWKEYE???. This technique showed focal retention of the
radiopharmaceutical in the upper right mediastinum, at the level of the top
of aortic arch.
Impression:
The scintigraphic finding is compatible with an ectopic parathyroid adenoma,
in the right upper mediastinum.
